8.5x11 fact sheet.

This publication is one of five success stories from 2015 showing the impact of Washington State's Healthy Communities program on local communities. This story describes how Peninsula Community Health Services found a way for patients who could not get to the doctor to get blood pressure checks and management through their local pharmacist.

This work was supported by the Washington State Department of Health, Office of Healthy Communities through the State and Local Public Health Actions to Prevent Obesity, Diabetes, Heart Disease and Stroke (1422) award, and the Prevention and Public Health Fund from the Centers for Disease Control and Prevention as part of the Affordable Care Act.
