Funding for the Healthy Communities Program comes from the following sources: Preventive Health and Health Services Block Grant, Comprehensive Cancer and Control, Diabetes Prevention and Control, Heart Disease and Stroke Prevention, Nutrition, Physical Activity and Obesity Prevention program andTobacco Prevention and Control.
